Reach

Bring prevention information and screening closer to families through the Center, community events, and partner locations throughout the Omaha metropolitan area.

Evaluate

Listen to the person’s questions and understand the screening or navigation need. This step does not replace clinical evaluation or diagnosis.

Look for Risk

Notice screening information or barriers that may need attention, then explain what is known and what still needs confirmation.

Advise

Share practical, understandable information and help the person identify questions and possible next steps.

Integrate

Connect the person with appropriate care or community resources when a referral pathway is available, using a warm handoff where the confirmed program supports it.

Sustain

Support continuity and follow-through when the confirmed program includes this service. The exact follow-up process depends on staffing, consent, event scope, and participating partners.

AIFS helps people understand what may come next without presenting the Center as the provider of diagnosis or treatment.

Warm referrals

When a confirmed pathway is available, AIFS can help connect a participant to an appropriate healthcare or community resource and explain the handoff. Specific provider relationships are not listed until confirmed.

Barrier resolution

Community Health Workers can help identify practical questions about language, forms, appointments, transportation, or system navigation. The specific support available depends on the program.

Closed-loop follow-up, where supported

RELAIS ends with Sustain: helping a person stay oriented to the next step when the confirmed program includes follow-up. AIFS does not promise a particular follow-up timeline, outcome, provider, or service outside the confirmed program scope.
